Overview
Health is more than the absence of disease, it is wellbeing of the whole person - physical, psychological, and sociological. The Health Clinic at Albertus Magnus College strives to meet the holistic needs of students in the undergraduate day program through the direct care of acute, chronic and preventive health needs; health education; coordination of care; referral to community resources and support of programs and initiatives that build a healthy campus community. The nurse run clinic is staffed by a Registered Nurse and a bilingual (Spanish) Advanced Practice Registered Nurse.
All services provided by the Health Clinic are free of charge*.
All services provided by the Health Clinic are confidential in compliance with HIPAA and FERPA guidelines. Patient consent may not be required if there is immediate intent to harm oneself or others. In the event of serious illness or injury, a parent/guardian will be notified at the discretion of the professional staff.
*Services provided by external providers as part of a student's assessment or referral will be billed by those entities directly; ex. Lab, imaging, urgent care, primary care provider, emergency room.

Tuesday 10:00 a.m. - 3:00 p.m., 3:30 p.m. - 6:30 p.m.

Services
Services provided through the health clinic may include:
- Acute illness and injury - ex. Cold or flu symptoms, sore throat, ear pain, abdominal pain, sprained ankle, wound care, urinary tract infection
- Chronic or recurrent health issues - ex. asthma, migraines, diabetes
- Preventive health/health education - ex. smoking cessation, diet/nutrition, physical fitness, sleep, stress management, sexual and reproductive health, acute/chronic illness
- Immunizations - immunization review, annual flu immunization clinic in the Fall
- Referral to outside providers - ex. Primary care provider, urgent care, specialist, hospital
- Referral to Counseling Services or Campus Ministry
- Coordination of care among providers
- Title IX confidential office
Student Health Requirements
All new and transfer students to the undergraduate day program are required to complete and submit:
- The Student Health Form Packet
- A copy of their valid insurance card (front and back). Please advise our office if you are enrolling in the insurance plan offered through Albertus Magnus College. Your card will be ready in August and January for Fall and Spring Semesters, respectively.
Deadlines
Requirements should be submitted by July 14th for fall semester and January 3rd for spring semester. Failure to submit required documentation will result in a hold on the student's account.
We recommend that you keep a copy of all forms for your records.
Physical Exam
Each student must have completed a physical exam within the last year.
Required Immunizations
In the past several years, there have been outbreaks of vaccine-preventable diseases on college campuses throughout the United States. Ensuring all students are in compliance with health requirements keeps our students and campus community healthy.
Prior to coming to campus:

MMR (Measles, Mumps Rubella) Immunization - If you were born after 12/31/1956, you must
provide proof of
- Two doses of MMR vaccine (one after your first birthday and one at least 28 days later) OR
- Two doses of separate Measles, Mumps and Rubella vaccines (the first dose on your first birthday and the second dose at least 28 days later)

Varicella (Chicken Pox) Immunization - If you were born after 12/31/1979, you must provide
proof of
- Two doses of Varicella vaccine (one after your first birthday and the second at least 12 weeks after the first dose (ages 1-12) and at least 4 weeks after the first dose if 13 years or older)
Prior to living on campus or participating in Athletics:

Meningitis (MCV4) Immunization - for A,C,Y,W-135
- PRIMARY AND BOOSTER DOSE OF MCV4. The most recent dose must be within 5 years of the first day on campus.
- Students who have their first MCV4 at age 16 or older need one dose within 5 years of the first day on campus.
Turberculosis (TB) Screeening
All students complete the TB Risk Assessment Questionnaire. Additional follow-up is determined based on risk, see form for details.
Influenza (Flu) & other Immunizations
The following vaccines are strongly recommend. Click each vaccine to learn more. Talk to your health care provider about your individual health needs to ensure you are up to date.
Annual Influenza Vaccine
Washing your hands and getting the flu vaccine are the two best ways to prevent getting sick with the flu. To help our campus community stay healthy, flu clinics are held on campus in the fall. The flu vaccine is covered by most insurance plans.

- If you choose not to be immunized against Hepatitis B you must print the Hepatitis B Waiver and submit it with your student health forms packet.
